Vaccination Information for Parents/Caregivers

The following information was distributed to all families with students 12 and over.

Dear Parents & Caregivers,

As you are aware, the Province has announced its Pfizer COVID-19 Vaccine Strategy for youth ages 12 and over starting in early June.

Northwest School Division has been asked to forward information from the Saskatchewan Health Authority (SHA) to you. The parent information package will be distributed in the next day. Any questions about the contents of the document should be directed to the SHA. If you have any questions regarding the clinic process or the Pfizer COVID-19 Vaccine, please direct your questions to the SHA by phoning 811.

The Consent Form in the package asks that you return the Form to your child's school. This is an SHA form and we have agreed simply to collect the forms on their behalf. The school will not be making a copy of the form and will give the forms directly to the medical personnel conducting the vaccinations. The medical personnel will discuss the Consent Form with your child when they are vaccinated.

Due to the limited number of doses allocated to in-school vaccinations, interested families will need to return the signed SHA Form with parental consent based on the chart below. The availability of drive-through, walk-in and pharmacy appointments in our region will have an impact on the number of students who wish to take advantage of the opportunity to be vaccinated at the school. Therefore, it is important to define the number of vaccines that will be brought to each clinic ahead of time. In schools, SHA staff will only be accommodating appointments made through the return of Consent Forms.

School based vaccinations are for students who have not received their first vaccine. Nurses will not administer second doses at these clinics.

NWSD is coordinating with the SHA the logistics and timeframes for the clinics. The SHA tentatively plans to offer COVID-19 vaccine clinics to the following schools on the dates below. Consent Forms should be returned by:

  • June 3 – Glaslyn, Pierceland
  • June 4 – CHS, TPEC
  • June 7 – Turtleford
  • June 8 – Marshall
  • June 10 – Goodsoil, Ernie Studer, Green Acre, H. Hardcastle
  • June 11 – Jonas Samson, Gateway
  • June 14 – Paradise Hill
  • June 15 – Hillmond
  • June 17 – Lashburn, JH Moore, CHS, St. Walburg
  • June 18 – Neilburg, Marsden
  • June 22 – Maidstone, Ratushniak

 

If clinic dates or times are not convenient for individual students, they may obtain a COVID-19 vaccine at:

  • Student Walk-in Clinic at the Meadow Lake Civic Centre on June 18;
  • Participating pharmacies administering the Pfizer brand;
  • Drive‐thru and walk‐in clinics administering the Pfizer brand;
  • Booking online for an SHA booked appointment; or
  • Calling 1‐833 SASKVAX (727‐5829) to speak to a booking agent
In order to assist us in planning please return your completed forms to the school - by June 1 for all clinics up to June 11, OR by June 7 for any clinics after June 12 - if you would like to have your child vaccinated at the above noted school vaccination clinic.
